Name Change Form
If your chosen name is different from your legal name, please complete the following form and submit to Jamie Childress, Dean of Enrollment at jchil111@cccc.edu.
Any name changes made the week prior to the start of the semester or during a semester will immediately change on class rosters, but will not update in Blackboard until the following semester.
Please be aware that by completing and signing this form you consent that all mailings sent to your home address will be addressed using your chosen name.
